There are many areas to consider when discussing the right care for our Residents. Often, that level of care changes as our Resident gets older, increases medication, modifies their nutrition plan or ignores other necessary changes needed to effectively age in place.
On admission to Johnson Creek Village, we prepare a customized care plan that is unique and beneficial to each Resident. It most likely will change as they change. We incorporate family history and knowledge to develop a roadmap for positive enhancement to each Resident's life. We will focus on the intricacies that make up their lives and that are most meaningful to them. We use their stories and history to develop a customized approach and routine that will promote a positive, uplifting and respectful feeling regarding their life stories and past experiences.
We use state of the art reporting tools and monitoring software that give us real-time information to manage the progress of our Residents. We document and communicate Resident's functions and spirit daily to insure our practice is on course and/or if modifications need to be made for their routines. This commitment reduces surprises and unknown behaviors which will provide peace of mind for the families and friends of our Residents.
Johnson Creek Village is proud to have an experienced geriatric RN reside on-site and be part of our clinical management team. We will work closely with other members of our Resident’s medical team to achieve desired outcomes. We will communicate regularly with loved ones and hold care conferences based on family and Resident needs. This keeps families alert to changes or any need for intervention or modification of their loved one's care plan.
Johnson Creek Village recognizes a brain healthy, balanced diet as a vital part of holistic senior living. We have linked relationships with local farms in our community to provide us with seasonal farm fresh foods. Under the guidance of Executive Chef, Buddy Haller, we will provide nutritious, fortified meals for our residents to enjoy. Some meals will feature food items grown on-site by our Residents and staff. We will also have a variety of healthy foods available at our snack bar.

It is used for depression, anxiety, dementia and more depending on the needs. It can release endorphins that produce a calming effect which can in turn help alleviate pain, reduce stress and improve your overall well-being.
